To provide participants with the knowledge and skills necessary to effectively undertake incident investigations, and to understand various techniques of accident / incident analysis.

 

On completion, participants will be able to demonstrate competence in the theory and practice of the following:

 

•  Conduct a fault tree analysis

•  Conduct a event tree analysis

•  Recognize undesirable events and consequences

•  Put in place mitigating control measures

•  Put procedures in place to control the undesirable event

•  Understand equipment failure modes

•  Identify contributing human errors

•  Identify equipment failures

•  Create graphical fault diagrams

•  Report Writing Techniques

 

There no pre-requisites for this course, although it is an advantage to have supervisory and management experience
